The Module "D" On-Line Training Session consists of the five (5) presentations below. You may click on any presentation in any order. Upon completing each presentation you will be asked to print out a certificate of completion or a question form that you will be required to attach to the cover sheet (Click Here for link to the "Module D" Cover Sheet) and turn in prior to being admitted to the Module "C" clinic session. NO Forms ..... No Admittance !!!! |
